Does My Child Qualify?
GAPP is for medically fragile children who need daily skilled or unskilled care to stay safely at home.
- Children ages 5–21 enrolled in Georgia Medicaid
- Children under 5 receiving physician-ordered skilled home nursing
- Conditions such as cerebral palsy, muscular dystrophy, spina bifida, seizure disorders, or tracheostomy / ventilator dependency
- Families who need daily skilled or unskilled support to keep their child safely at home
Accepted plans: Traditional Georgia Medicaid, Amerigroup, CareSource, Peach State Health Plan. Common qualifying conditions include cerebral palsy, muscular dystrophy, spina bifida, seizure disorders, and tracheostomy or ventilator dependency.

We Handle the Paperwork With You

Confirm Medicaid & Eligibility
Your child needs Georgia Medicaid. We check whether their age, diagnosis, and care needs meet GAPP criteria.

Gather Documentation
Medicaid ID, current medical diagnosis, demographic information, a physician referral, and signed medical forms. We help you assemble all of it.
- 03
Nursing Assessment & Plan of Care
A ReadyMed nurse completes an assessment and builds the Plan of Care that Medicaid requires.

Submission & Approval
We submit everything to Medicaid. Approvals can happen in as little as 3–4 weeks.
- 05
Care Begins
Your assigned nurse or aide starts scheduled in-home care, with ongoing clinical reviews and case management.
