Children ages 5–21 can qualify for unskilled services. Children under 5 require skilled home nursing care and must currently be receiving physician-ordered services.

Once we have the required documentation, approvals can happen in as little as 3–4 weeks.

Traditional Georgia Medicaid, Amerigroup, CareSource, and Peach State Health Plan.

Your child’s Georgia Medicaid ID, a current medical diagnosis, contact and demographic information, a physician referral with signed medical forms, and a nursing assessment and Plan of Care. We help you assemble all of it.

Placements can often be made within 24–72 hours, depending on availability and needs.

Personal care homes, assisted living, memory care, and skilled nursing facilities — all licensed and pre-vetted.
